Start here when something changed

Care has been denied, delayed, or is ending. Start with what happened.

Tell us what happened in your own words. We’ll help you make sense of the notice, see what is time-sensitive, and find the next step.

Start with a few details, or add the Medicare notice if that is easier.

What happened?

For example: “My mother’s rehab says her Medicare coverage ends Friday.” Please don’t include names or Medicare numbers.

Describe what’s happening above, then choose Show me the path. We’ll help you sort out what to look at next.

Have the Medicare notice in front of you?

You can add the PDF or a photo here if you want the document beside you while you work through the questions. In this preview, the file stays on your device and is not sent or stored anywhere.

PDF, JPG, JPEG, PNG, or HEIC. Preview mode only: the selected file remains local to this browser session.

No document selected. You can also continue without one.

Notice Decoder

If you would rather not add the document, that is fine. Read a few standard fields with us and the Notice Decoder can still help identify the notice and the time-sensitive path.

1. Read the title line at the very top

This single line identifies most notices. If you can’t find it, choose “I’m not sure” and answer the rest.

2. What kind of care is it about?
3. At the top, is there a private health-plan name or logo?

A plan name/logo (for example Humana, UHC, Aetna) usually means Medicare Advantage; just Medicare or the provider usually means Original Medicare.

4. Is there a specific calendar date your coverage or services will END?

A care date only. This is not personal identifying information, and it stays in your browser. Leave blank to keep it general.

Answer the questions above and choose Identify the notice. Your result will appear here.
